Please bring your insurance card and photo identification. If you are due to complete any forms these will be emailed to you ahead of time through our intake service. Please complete these forms online ahead of your appointment. If these forms are not completed before you arrive, your appointment may be delayed while you do forms in our office. Being prepared really helps with the flow of patients in our office. We request this information from our patients to provide you with the best medical care possible. These forms are necessary because our doctors request your most recent health history and we are required to update your insurance information annually.
As a new patient coming to our clinic, it is important to let you know that any records you bring to our clinic cannot be copied or transferred from us to another office or hospital. If you want copies, please make copies for yourself before giving your records to our office. If your appointment pertains to an operative consultation, please bring any previous operative records with you.
